CBT for ADHD · Online India · Gurgaon

CBT for ADHD: less willpower, more systems.

CBT for ADHD is a structured, skills-based form of cognitive behavioural therapy adapted for how ADHD brains work. It builds outside structure first (planning, reminders, breaking tasks down) and then works on the procrastination thoughts and shame that keep you stuck. Online across India and at our Gurugram clinic.

How It Is Different

CBT for ADHD is not regular CBT.

Standard CBT assumes you can remember homework, stick to a plan and change behaviour mainly by changing thoughts. With ADHD, the problem is rarely knowing what to do. It is doing it at the right time. So CBT for ADHD flips the order.

🗂️First

Structure before insight

We set up external systems first: one planner, visible reminders, fixed review times. Thinking work comes once there is something stable to build on.

🧩Small

Tasks shrunk to the first step

Big tasks are broken down until the first step takes under ten minutes and is physical and specific, such as "open the laptop and name the file".

💬Thoughts

Targeting ADHD-specific thoughts

We work on the thoughts that feed procrastination ("I'll do it when I feel like it", "it has to be perfect") and the shame that follows missed deadlines.

🔁Practice

Missed homework is information

If something does not happen between sessions, we treat it as data about what needs to be easier, not as a sign you did not try.

What It Covers

What a course of CBT for ADHD looks like.

Structured CBT programmes for adult ADHD usually run for about 12 to 16 weekly sessions, each building one skill and practising it in your real week. The order is adapted to you, but most courses cover the areas on the right.

If emotional sensitivity or rejection sensitivity is a big part of your ADHD, we bring in emotion-focused and compassion-based work alongside CBT. See ADHD treatment and therapy for the full range of approaches.

  1. Understanding your ADHD: how it shows up for you specifically.
  2. Organisation and planning: one trusted system for tasks and time.
  3. Managing distractibility: matching task length to attention span, setting up your environment.
  4. Starting tasks: beating task paralysis and procrastination.
  5. Adaptive thinking: catching and answering unhelpful thoughts.
  6. Emotions and setbacks: frustration, overwhelm, and the "what is wrong with me" spiral.
  7. Keeping it going: what to do when the systems slip, because they will.
Try This Week

Five CBT exercises for ADHD you can start today.

These are simplified versions of tools we use in sessions. They work best when you pick one, not all five.

⏱️Starting

The ten-minute first step

Write the very first physical action of a task you are avoiding, then set a timer for ten minutes and do only that. Stopping after ten minutes is allowed.

📌Planning

When-then plans

Link a task to a fixed cue: "When I sit down with my morning tea, then I check my planner." Cues do the remembering so you do not have to.

📓Systems

One place for everything

Choose a single planner or app for every task and appointment, and spend five minutes with it at the same time each day.

⌛Time

Make time visible

Use a visible timer or clock for tasks. Time blindness is easier to manage when you can see time passing.

🧠Thoughts

Catch the procrastination thought

When you avoid a task, write down the thought ("I need to be in the mood"), then write a more useful one ("I only need to start, not feel ready").

🎯Review

Two-minute evening review

Each evening, note one thing that worked and pick tomorrow's single most important task. Small, daily and kind.

The Evidence

Does CBT work for ADHD?

Yes, for many adults and teenagers. Randomised controlled trials of CBT adapted for adult ADHD have found improvements in symptoms and daily functioning, in people taking medication and in those who are not. Clinical guidelines, including the UK's NICE guideline on ADHD, include CBT-based psychological treatment as an option for adults. It is not a cure, and it does not replace medication when symptoms are severe, but it teaches skills that medication cannot.

For younger children, behavioural parent training usually works better than CBT alone. See ADHD therapy for children.

Your first session

  1. What is getting in the way at work, at home and with yourself.
  2. What you have tried, including past therapy, apps and planners, and why they stopped working.
  3. Two or three goals we can measure, like starting work before 11am.
  4. One small experiment to try before the next session.

No diagnosis yet? You can start therapy, or book an ADHD assessment first.

Questions

CBT for ADHD: common questions.

Does CBT work for ADHD?
Yes, for many adults and teenagers. Randomised controlled trials of CBT adapted for adult ADHD have found improvements in symptoms and day-to-day functioning, both in people taking medication and in those who are not. It works best when it is practical and skills-based rather than talk-only. It is not a cure, and some people also need medication.
How many CBT sessions are needed for ADHD?
Structured CBT programmes for adult ADHD usually run for about 12 to 16 weekly sessions. Many people notice practical changes within the first six to eight. Some continue with less frequent sessions afterwards to keep new habits going.
Is CBT better than medication for ADHD?
They do different jobs. Medication can reduce core symptoms such as inattention and impulsivity, while CBT builds the skills and systems that medication does not teach, like planning, starting tasks and handling setbacks. Many adults do best with both. A psychiatrist can advise on medication.
Can CBT help children with ADHD?
For younger children, behavioural parent training and behaviour therapy are usually more effective than CBT on its own. CBT-based approaches become more useful in the teenage years. See our page on ADHD therapy for children.
I tried CBT before and it did not help. Why would CBT for ADHD be different?
Standard CBT often assumes you can remember homework, stick to plans and change behaviour by changing thoughts. CBT for ADHD starts from the opposite assumption: it builds external structure first, keeps tasks small and concrete, and treats missed homework as information rather than failure.
What is the 30% rule for ADHD?
It is a rule of thumb described by psychologist Russell Barkley: a person with ADHD may function, in executive skills like self-control and planning, around 30 percent behind others of the same age. It is not a precise measure, but it helps parents and adults set expectations and build in more support rather than blaming effort.
Can I do CBT for ADHD online in India?
Yes. CBT for ADHD works well online because it is structured and practical. We offer it online across India and in person at our clinic in Sector 70, Gurugram.
